To nominate either complete the online form below or download and fill in the nomination form and send it to us along with the requisite nomination fee. If you do not receive acknowledgement of your nomination within three hours plase contact us on (01) 7008508

Closing date for Nominations  – 29th February 2012

  1. Choose the category of award for which you wish to nominate yourself, a colleague or a team. If you wish to make an entry for more than one category, you must fill in a separate entry form for each category of award.
  2. Read the judging rules carefully and prepare a summary (maximum 250 words per section) of why you wish your entry to be considered.
  3. Complete the entry form and send together with all relevant supporting documentation to: ‘Biomnis Healthcare Innovation Awards, c/o Investnet Limited, The Tony Ryan Academy, 3013 Lake Drive, Citywest Business Campus, Dublin 24’.  For enquiries Telephone: +353 (1) 7008508.
  4. Electronic entries MUST be submitted to: nominations@healthawards.ie
  5. There is a nominal administration charge of €25 for all entries before December 31st and €50 there after, payable by cheque or credit card.
  6. Give as much information as you can with emphasis on patient outcomes.
  7. We regret that we are unable to accept clinical trials until patient outcomes are proven.

Rules:

  1. Open to all people working in or otherwise contributing to the Irish Healthcare Services Sector.
  2. An individual, project or team may be nominated for one or more of the nine awards, though any one project will not be selected as a winner in two award categories.
  3. Each entry must be in the name of an individual or a team. In the case of a team, one individual must be identified to represent that team.
  4. All entries will be treated as confidential. Any requests for media access will be passed on to the nominees, who are to decide on any appropriate access.
  5. Closing date for entries: 29th February 2012 Entries will be acknowledged by return.
